1:1 Chromebook Insurance Plan

Insurance Forms
Insurance forms will be sent home to parents/guardians. If an additional copy is needed, a link to download the form is below. Students are required to have a completed form with them on the day they are scheduled to pick up their Chromebooks. No devices will be distributed without a completed form.
Insurance
It is strongly recommended that every student’s device be insured through the United School District insurance plan, as part of participating in the 1:1 Chromebook Initiative. Students and parent(s)/guardian(s) are responsible for maintaining this equipment in the condition in which it is issued. Parent(s)/guardian(s) and students will incur all expenses resulting from damaged or lost equipment or attempts to tamper with equipment. Please note that this coverage does not carry over from year to year if it goes unused. Students with a free/reduced lunch designation will have their insurance fee waived.
 
Pricing Guidelines for Repairs and Replacements
The following chart of repair costs is provided as a reference to parents/guardians when making a decision on insurance coverage.

For repairs not listed above, cost, and applicable insurance claims will be determined by the cost of replacement parts to the district at the time of repair. If insurance coverage is not purchased and the Chromebook is damaged, the parent(s)/guardian(s) will be invoiced for the cost of repairs. The United School District accepts payment in the form of check or money order.
 
Insurance Plan Options
  1. Individual Student Plan - $40
    • Cost is a non-refundable payment of $40.00 per school year per device
    • Covers one student and one Chromebook
    • Coverage includes up to two claims per coverage year
  2. Family Plan - $80
    • Cost is a non-refundable payment of $80.00 per school year
    • Covers all students and assigned Chromebooks within a single household
    • Coverage includes up to four claims per coverage year
Payment
Payment for the 2023-2024 school year is due at the time of the Chromebook distribution. No payments will be accepted after this date. Students with a free/reduced lunch designation will have their insurance fee waived once lunch designation is verified.
Please print your student’s first and last name in the memo portion of your check or money order. Please make the check or money order payable to the “United School District”.
 
Coverage
This insurance plan covers the Chromebook loaned to the student against two incidents of accidental damage. The following items are covered:
  1. Accidental damage
  2. Environmental damage from Natural Disasters, Fire, or Power Surges
  3. Theft - All theft claims must have a police report filed within 5 days of the theft
  4. Vandalism
  5. Lost or Damaged Chromebook Charger
Procedures for Damages
  1. Individual or Family Insurance Plan purchased
    1. Parent/Guardian/Student will complete a Repair Request Form and turn in to school office.
    2. Technology Department will assess damage and prepare applicable insurance claims necessary to perform repair.
    3. Parent/Guardian/Student will receive a Repair-Insurance Claim form with description of damage and applicable insurance claims needed to perform the repair.
    4. Parent/Guardian/Student will sign and return the Repair-Insurance Claim form to the school office.
    5. Device will be repaired and released back to the student.
  2. Individual or Family Insurance Plan purchased, but repair is not covered or insufficient claims remaining
    1. Parent/Guardian/Student will complete a Repair Request Form and turn in to school office.
    2. Technology Department will assess damage and prepare cost to perform repair.
    3. Parent/Guardian/Student will receive a Repair-Insurance Claim form with description of damage, explanation of why the damage was deemed not to be covered, and cost to perform the repair.
    4. Parent/Guardian/Student will sign and return the Repair-Insurance Claim form to the school office.
    5. Device will be repaired and released back to the student.
  3. Individual or Family Insurance Plan not purchased
    1. Parent/Guardian/Student will complete a Repair Request Form and turn in to school office.
    2. Technology Department will assess damage and prepare cost to perform repair.
    3. Parent/Guardian/Student will receive a Repair-Insurance Claim form with description of damage and cost to perform the repair.
    4. Parent/Guardian/Student will sign and return the Repair-Insurance Claim form to the school office.
    5. Device will be repaired and released back to the student.
Loaner Devices
Loaner devices may be available when assigned devices are being repaired, left at home, or if a child has chosen not to participate in the 1:1 initiative.
These devices may be picked up in the morning at the school library and can be checked out to the student as needed but must be returned at the end of the school day and are in limited quantities and will be distributed on a first-come, first-serve basis.
Students and parent(s)/guardian(s) are responsible for maintaining loaner equipment in the condition in which it is issued and will be responsible for any damages incurred while checked out.
